Notice of Availability of Form 1095-C

Hard copies are available upon request only. To request a hard copy of your Form 1095-C, you can submit a request to Human Resources through the following methods:

New for 2024, the Paperwork Burden Reduction Act (PBRA) of 2024 modified provisions under the Patient Protection and Affordable Care Act so that employers are no longer required to send forms to covered individuals showing proof of minimum essential coverage (IRS 1095-C forms) unless a form is requested. Please note, requests for hard copies will be fulfilled within 30 days from the date the request is received.
